How To Schedule An Appointment
Appointments are scheduled according to the availability of a particular
therapist's schedule and is handled by calling our office manager, Karen,
at 614.451.3877. She will arrange a mutually agreeable time to meet. Arrangements
for payments of fees, insurance forms, and other required paperwork will
be filled out prior to the first session. Generally, Karen can give you
an approximation of what your co-pay will be, if you are covered under
one of the insurance companies with which we are contracted. If we are
not on panel with your particular insurance carrier, the fees for services
will be due at the time they are rendered. We accept cash, check, MasterCard,

Data That Will Be Collected
Each patient will complete a computerized psychosocial history, a brief
personality inventory, and the Quality of Life Index. This information
is entirely confidential and becomes a part of your record. To read

click here. The information gathered from these
inventories are invaluable in helping your therapist understand your
presenting problem and formulating the appropriate treatment plan with
you. This usually takes place before your initial diagnostic evaluation
with your assigned therapist. These inventories take approximately 75
minutes to complete.

Emergency / After Hours
Emergencies are handled on a case-by-case basis with your therapist.
Your therapist can be contacted in the event of an emergency by dialing
614.451.3877, accessing your therapist's voice mail box, leaving a message,
including your name and telephone number, and pressing 8 after
leaving your message. Your therapist will be contacted immediately and
should return your call as soon as possible.
If the emergency is life-threatening, please go to the
nearest emergency room or dial 911.
